Vietnam visa application form

From March 1st, 2015 the old visa application form Vietnam is replaced by the new one. This is important document requirement when you enter or exit in Vietnam Immigration Counter.

If you already had the Vietnam application form, please ignore it and download the new application form by the following link: Vietnamese Visa Application Form.pdf. This is the newest form which allowed by Vietnam Immigration Department .

The visa Vietnam application form has three sections:

Visa information

• Number of applicants: 10 applicants are maximum for each application
• Type of visa: there are 4 types of visa for you: one month single, one -month multiple, 3- month single, and 3- month multiple.
• Date of arrival: enter your intended date of entry into Vietnam
• Port of entry: Noi Bai Airport, Danang, Cam Ranh, Phu Quoc and Tan Son Nhat Airport are 5 Vietnam’s international airports at present

Personal information: Your full name, date of birth, passport number, nationality, and gender should be provided exactly what it is shown on your passport.
